Care One Rehabilitation Services is a company dedicated to provide compassionate high quality Home Health care. Care One provides a full range of nursing services, therapy, home health aides.Services are covered by many commercial and long-term-care insurance companies, Medicare, and Medicaid.

Love, patience, understanding, and the familiar surroundings of family and friends, along with Care One support services, help many clients recover faster. Home care may result in lower costs than institutional care, and at Heartland we work with you to provide the services you need at a reasonable rate.

Care One also offers health education to senior clients and their families with emphasis on diabetes management and cardiac rehabilitation. In-service education for community organizations are also provided.

Care One Home Health Care may accept or make arrangements to coordinate patient care through the following insurance programs:
