Care One Rehabilitation Services provides care for the client who is ill, convalescing, disabled or aged, and needs intermittent skilled visits. All services are performed under the direction of the physician, which can be coordinated through the hospital discharge planning department, the insurance company or the physician's office.

• Case Management in collaboration with attending Physician and other Health Care Professionals
• Infusion Therapy(Antibiotic, Hydration, TPN)
• Blood Pressure, Blood Sugar Monitoring
• Laboratory Monitoring
• Pain Management
• Patient/ Family Education or Teaching, Supervision, and Evaluation of Plan of Care
• Wound Care Management
• Disease Process Management
• Urinary Catheter/Peg Tube Management

Care One Rehabilitation Services, Inc. prides itself in ensuring continuity of care by working closely with the client's physician. Services are coordinated to meet the individual needs of the client and the physician's plan of care.

A registered nurse is on-call 24 hours a day, seven days a week. Care One serves clients in the counties of Oakland, Wayne and Macomb.
